Colorado Connections is a statewide program of Denver Options that provides care coordination to adults with traumatic brain injury (TBI).

Our care coordination system draws from Denver Options' experience and excellence in service coordination. Colorado Connections' care coordinators work with customers to find the best care and support systems to meet their individualized needs.
