Individual Family Service Plan

The Individualized Family Service Plan is a plan of service that outlines services that will help your child achieve success and reach learning goals. Services are determined by a child’s needs and are provided in the home.

The Individualized Family Service Plan (IFSP)

• A plan of service tailored to your child’s needs

• States how outcomes (goals) for your child will be reached in everyday routines and activities

• Outlines the services to be provided

• Determines the best way to fund needed services

• The IFSP will be reviewed every six months and re-written annually

• You have the right to make changes to your child’s plan when you feel changes are necessary
